Chronic Disease Management

At Mandurah Medical Centre all of our GP’s have a keen interest in Chronic Disease Management and offer comprehensive care of our patients.

Our Doctors throughout the years, have developed the necessary and critical hands on skills to diagnose and manage conditions such  as Asthma, Diabetes, Osteoporosis, High Blood Pressure and High Cholesterol.

Typically, chronic diseases can range from mild conditions such as short-sightedness, to debilitating conditions such as arthritis and back pain, and to life threatening heart disease and cancers. These condition may never be cured completely, so there is generally a need for long term management.

Through our “Better Health Program”, we also offer excellent tailored GP Management Care Plans which aim to guide and educate patients in maintaining control of their conditions and future risk factors of others.

In collaboration with your GP, Team Care Arrangements referrals may also be included to assist in improving, maintaining and further educating patients with their ongoing conditions. Allied services include but are not limited to Podiatrists, Dieticians, Physiotherapists, Exercise Physiologists and Diabetes Educators.

  • Smoking Discussion, Alcohol Advice
  • Weight Check
  • Dietary Advice
  • ECG (as required) and Blood Pressure measurements
  • Physical Activity Advice
  • Cardiovascular Risk Check
  • Spirometry Test (as required)
  • Cholesterol Check
  • Diabetes Screen
  • Results of Blood Tests
  • GP consult and review
  • Information and advice
